Healthcare Solutions Centers, LLC is a healthcare services organization that provides clinical services, preventive care, wellness programs, occupational health services, employer-sponsored onsite or near-site clinic services, and related healthcare offerings.
Services may be provided in-person, onsite at an employer location, through mobile clinics, or via telehealth where permitted by law.

- Appointment availability is not guaranteed.
- Missed appointments or late cancellations may be subject to fees as permitted by law and employer agreements.
- HCS reserves the right to reschedule or cancel appointments due to provider availability, emergencies, or operational needs.
- Payment, Fees, and Insurance
Payment terms may vary based on service type, employer sponsorship, insurance coverage, or self-pay arrangements.
You agree that:
- You are responsible for all charges not covered by insurance or employer agreements;
- Copayments, deductibles, and self-pay fees may be due at the time of service;
- HCS does not guarantee insurance reimbursement.
Failure to pay outstanding balances may result in suspension of non-emergency services.
- Employer-Sponsored and Onsite Clinic Services
For employer-sponsored or onsite clinic services:
- Access to services may be limited by employer eligibility rules;
- Services are provided independently of employer employment decisions;
- Employers do not have access to individual medical records or protected health information, except as permitted by law.
- Telehealth Services
Telehealth services are provided in accordance with applicable state and federal laws.
You acknowledge that:
- Telehealth may not be appropriate for all conditions;
- Technical failures may impact service delivery;
- In-person care or emergency services may still be required.
- Privacy and HIPAA Compliance
HCS complies with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws.
Protected health information (PHI) is used and disclosed in accordance with our Notice of Privacy Practices, which is available upon request and through our website.
By using our services, you acknowledge receipt of our Notice of Privacy Practices.
- Electronic Communications & Text Messaging
By providing your phone number and/or email address, you consent to receive communications from Healthcare Solutions Centers, LLC, including appointment reminders, care coordination messages, billing notifications, and administrative communications via phone call, text message, email, or patient portal.
You understand that standard text messaging and email may not be fully secure and may carry a risk of unauthorized access. You acknowledge and accept these risks and agree that Healthcare Solutions Centers, LLC is not responsible for unauthorized access resulting from your chosen communication method.
You may opt out of non-essential electronic communications at any time by notifying our office in writing; however, communications related to your care, scheduling, or billing may still be required.
- Patient Portal Use & Security
If you use a patient portal provided by Healthcare Solutions Centers, LLC, you are responsible for maintaining the confidentiality of your login credentials. You agree to notify us immediately of any suspected unauthorized access.
Healthcare Solutions Centers, LLC is not responsible for breaches resulting from patient failure to safeguard login credentials, use of shared devices, or access by unauthorized individuals.
- Employer-Sponsored and Onsite Clinic Services
For employer-sponsored or onsite clinic services:
- Access to services may be limited by employer eligibility rules;
- Services are provided independently of employer employment decisions;
- Employers do not have access to individual medical records or protected health information, except as permitted by law or authorized in writing by the patient.
Aggregate, de-identified data may be shared with employers for reporting, quality improvement, and program evaluation purposes.
- Authorized Representatives and Third Parties
You may authorize another individual to receive information or pick up medications on your behalf by completing appropriate authorization forms.
HCS is not responsible for misuse of information or medications once released to an authorized third party.
- Website and Portal Use
You agree not to:
- Use our website or systems for unlawful purposes;
- Attempt to gain unauthorized access to systems or data;
- Upload malicious software or interfere with system operations.
HCS may suspend or terminate access for violations of these Terms.
- Intellectual Property
