Your on-site clinic is here to make healthcare simple, convenient, and completely accessible. Located right at your workplace, the clinic provides high-quality medical care without long waits, urgent care visits, or months of scheduling delays. All visits are free for employees and eligible spouses and dependents on the UMR medical plan— there are no copays, ever. Your medical information is completely confidential. The clinic operates under strict healthcare privacy laws, and your employer does not have access to your personal health details.
Please note that the Bill Luke Tempe location currently only offers phlebotomy services once a month.

How to Complete Your Biometrics Review
A biometric screening is a quick, convenient way to get a snapshot of your overall health. This three step program includes annual lab work, such as cholesterol, blood sugar, and other key markers, followed by a brief mini physical with a provider. During your visit, we will review your results, check vital signs like blood pressure and BMI, and discuss personalized recommendations to support your health and wellness goals. It is a simple, proactive way to catch potential concerns early and stay on track with your health.
You can participate once a calendar year in this program.
Step 1: Request Two Appointments
Request two appointments, one for a lab draw and one for a biometric review a week after the lab draw appointment. You can request using the scheduler below or call HCS at (602) 424-2101.
Step 2: Attend Both Appointments
The biometric review appointment will include a physical exam and a review of your labs such as cholesterol, blood glucose, and other key health indicators. You will receive a Bill Luke Biometric Review Form form your provider at the end of the appointment if you met all program qualifications.
Step 3: Submit For Your Incentive
Submit your completed Bill Luke Biometric Review Form to your HR representative to receive your $200 incentive. HCS will never submit any forms to HR for you.
How to Complete Other Incentives
Take advantage of your preventive care benefits and earn rewards while prioritizing your health. Eligible screenings and immunizations, including colonoscopy, mammogram, CT lung cancer screening, and recommended vaccines such as influenza, shingles, and pneumococcal, each qualify for a $100 incentive. These services are designed to support early detection and prevention, helping you stay ahead of potential health concerns while receiving added financial rewards for completing important wellness steps.
Incentives are generally limited to once per year, though some may have longer eligibility timeframes based on individual patient factors. Please review your specific requirements with your provider to ensure you qualify. An example of this is a colonoscopy which can go as long as every 10 years.
Step 1: Complete Your Incentive
Complete an eligible screening or immunization with Healthcare Solutions Centers (HCS), your primary care provider, or a pharmacy. If completed outside of HCS, please bring documentation showing the service was performed. If you would like to complete with HCS, please request an appointment with us at the top of this screen.
Step 2: HCS Review & Verification
Our team will review your records to confirm the incentive requirements have been met for the current year. If we ordered radiology for an incentive, results will come back to us directly. If you completed a vaccine at a pharmacy, please have them fax us the vaccine records.
If you completed with an outside provider, we will need to review to verify you qualify for the incentive. Once verified, we will provide you with a completion form.
Step 3: Submit to HR
Submit your completed Bill Luke Completion Form to your HR representative to receive your $100 per incentive. HCS will never submit any forms to HR for you.

All employees can be seen at the clinic.
Any spouses or dependents that are on the UMR Bill Luke medical plan can also be seen at the clinic.

Does it cost money to be seen?
All clinic visits are provided at no cost to eligible patients.
If you choose to participate in the semaglutide weight loss program, there will likely be a cost associated with the medication.
Additionally, services completed outside of the clinic—such as pharmacy, laboratory, or radiology services—may result in separate charges.

What is the Biometric Program?
Visit 1: Blood Draw
You will come to the clinic for a quick lab draw to check important health markers such as cholesterol and blood sugar. This is completed right here at the clinic.
Visit 2: Mini Physical & Results Review
You will return for a brief appointment where we check your blood pressure, height, weight (and other required measurements), and review your lab results with you. During this visit, we will explain your numbers and answer any questions you may have.
After completing both visits, you will receive a printed confirmation stating that you successfully completed the biometric program. You simply submit this form to your employer, and they will issue your $100 incentive.
Your personal health results are completely confidential. This screening is for wellness purposes only and does not include drug testing (including marijuana).
When can I get my labs drawn?
On-site phlebotomy is offered at clinic locations at least once per month.
- Bill Luke Phoenix: the 4th Monday of each month from 8:00am-10:00am by appointment.
- Bill Luke Santan: the 3rd Wednesday of each month from 7:30am-9:30am by appointment.
- Bill Luke Marana: the 1st Wednesday of each month from 7:30am-9:30am by appointment.
- Bill Luke Tempe: the 1st Wednesday of each month from 8:00am-10:00am by appointment.
In addition, weekday lab draws are available by appointment at the Healthcare Solutions Corporate clinic in Central Phoenix off of 11th street and Camelback road.
Patients also have the option to complete their lab work at any Sonora Quest Laboratories location for convenience.
